Who Is Eligible?
A basic life insurance policy is available to
all full-time employees at no cost to the employee.
Benefit amounts vary based on the
unit affiliation of the employee.

Supplemental Life Insurance
An additional $10,000 increments of supplemental life insurance
may be purchased by the employee. The employee may purchase up to
five (5) times their salary. Employess may cover spouse
and children up to 19 or 26 years of age if a full time student.
Premium amounts vary based on the age of the employee.
Rates increase as employees enter new age categories.

Payment
Supplemental life insurance premiums are deducted from
employee paychecks over the course of twenty (20) pay periods.

Declining Life Insurance
If a full-time employee declines basic life insurance
they must sign the enrollment form.
A witness must also sign the form.

Cancellation of Supplemental Life Insurance
If an employee wishes to cancel the $10,000 supplemental
life insurance policy, employee must indicate this request
in writing and forward it to the Human Resources office.
A copy will be placed in their file and another copy
will be forwarded to Payroll.

Picking Up Basic or Supplemental Insurance After
Dead line Has Passed or Declination of Coverage

Any employee wishing to pick up insurance at a later date
must provide evidence of insurability prior to
the granting of any life insurance coverage.

Portablility
If an individual resigns or is terminated,
The individual has 31 days to decide to continue coverage.
The insurance company will bill directly.
JPS will not cover the cost of previous district paid life insurance
if the individual decides to PORT Life Insurance.
